If you want to be a caregiver in Oklahoma, you’ve got to hit a few marks. First off, you need to be at least 18 and an Oklahoma resident, which you can prove with a current state license or ID. You also need to show you’re officially designated by the patient to help them out, but the good news is there’s no fee for the caregiver card.

Patients who need help with their medical marijuana can only designate a caregiver if their doctor’s recommendation says they need one. You’ll need to submit the Adult Patient Caregiver Designation Form signed by the patient, or if it’s a minor, the Minor Patient Caregiver Designation Form signed by a parent or guardian. Minor patients can have up to two caregivers who are parents or legal guardians. The caregiver license doesn’t have an application fee and is valid until the patient’s license expires. Here’s what you need to qualify:

  • digital copy of proof of Oklahoma residency

  • digital copy of proof of identity

  • a clear, color, full-face digital photograph valid for use in U.S. passports

  • a submitted appropriate Patient Caregiver Designation Form signed by the patient
